Engine failure caused by a broken gear system led to a helicopter crash that killed a gas pipeline inspector in Co Clare last month, a report revealed today.

Frederick Balcombe (69) from Skerries, Co Dublin, was with a colleague on a low-level survey flight for Bord Gais when it crashed into a field near the village of Ballynacally, about 10 miles from Ennis. The pilot survived the crash.

The pair had been on a flight checking lands close to the Galway-Shannon gas line.

Inspectors from the Transport's Air Accident Investigation Unit (AAIU) opened an inquiry and uncovered a fault in the gearbox.

Their preliminary report found the bevel gear had disintegrated, cutting off the fuel supply and causing engine failure. The AAIU said it could not determine what caused the gear system to fall apart.

Investigators have asked the Irish Aviation Authority to review whether single-engine helicopters are suitable for low-level operations due to increased hazards.

A final AAIU report will be published later.
