A file has been sent to the Director of Public Prosecutions after an investigation into the death of a patient at a Dublin hospital.
The patient, a British man in his mid-30s, died at the Central Mental Hospital, Dundrum, on September 12th, 2001, a few hours after an incident in which he was allegedly restrained by staff. A post-mortem was carried out on the man's body by the Deputy State Pathologist, Dr Marie Cassidy, before it was returned to Britain for burial. The patient's death was the subject of investigations by the Garda and by an independent team requested to report on the incident by the hospital.
Now it has emerged both investigations are complete and a file has been forwarded by the Garda to the DPP.
