The man who was heading the investigation into the death of an elderly patient at a north Cork hospital last month has decided to step aside from the investigation.

Hannah Comber (65) died in the Heatherside Hospital on the outskirts of Buttevant, Co Cork on June 22nd last. Ms Comber, from Charleville, became entangled in a strap on a specialised therapeutic chair at the hospital but no mention that the patient was caught in the strap was made by staff when they informed gardaí of her death.

Mr Tom O'Dwyer was appointed chairman of the investigation committee appointed to examine the circumstances surrounding the death of the pensioner.

In a statement, the Health Service Executive (HSE) South said Mr O'Dwyer decided to step down after concerns were raised about his independence because of his past management involvement with the former Southern Health Board.

"While it is regrettable, the HSE respects Mr O'Dwyer's decision and his wish that nothing would interfere with the completion of the work of the investigation team. The HSE appreciates Mr O'Dwyer's commitment and his assistance."

Mr Pat Gaughan has agreed to assume the role of chairman. He is a former chief officer of the Midland Health Board.
