Mining and quarrying experienced an unprecedented six fatalities last year;

Almost one-quarter of local authorities inspected found to be in "limited or no compliance"

with health and safety obligations; and

The most common fatal incident types involved victims being "trapped or crushed by falling object or machinery" (14 deaths), "struck by falling/moving/flying objects" (12 deaths) and falling from heights (10 deaths).
