Sir, – So "Heads will roll" in the health service ("Varadkar warns on overcrowding", September 15th)? How could heads roll considering that whenever anything goes wrong or is the subject of criticism in the health service, it always seems to be because of some "systemic" issue? It's never because someone identifiable has been or can be identified as having caused it. It's because of the "system". And no doubt we will always "take steps to learn from the systemic failing so as to ensure it never happens again". It's as though systems just happen out of the blue with nobody responsible and nobody accountable. There always seems to be a chain of people involved in the system so you can't individually identify who was the cause. – Yours, etc,
