Werder had taken a deserved lead when full back Christian Schulz got the final touch on an untidy opening goal six minutes before the break, but they missed chances to build a commanding lead and Juventus hit back in ruthless style.
Pavel Nedved converted a through ball from Patrick Vieira for the equaliser in the 73rd minute and David Trezeguet's header with eight minutes to go looked set to give the Italian champions victory.
Tim Borowski's near-post flick levelled the scores again in the 87th minute, however, and Bremen completed a memorable comeback when Johan Micoud scrambled the ball home from a corner in the fourth minute of injury-time.
Werder still have plenty to do, but last night they could bask in a win they fully deserved. Thomas Schaaf's side lost 10-2 on aggregate to Lyon at the same stage of the competition last year.
