The Robins took a shock lead on 23 minutes, David Noble threading past Boro keeper Mark Schwarzer, and it stayed that way until the 69th minute, when substitute Mark Viduka nodded home a Stewart Downing cross to equalise.
The tie went into extra-time, and in the 96th minute Yakubu won then missed a penalty, but made amends six minutes later by giving Boro the lead.
City made it 2-2 just three minutes from time through skipper Jamie McCombe to take it into penalties.
Boro came through the shootout 5-4 on sudden death, but not before Yakubu missed his second penalty of the game. Adam Johnson hit the winning kick for the hosts, after Craig Woodman and Phil Jevons missed, and Gareth Southgate's men will play West Brom in the fifth round on Saturday.
